Dear Parent/Carer

If your child(ren) has tested positive for COVID19 in the last 12 weeks, government guidance has changed and they are now NOT eligible to be vaccinated this Friday.

Further dates may be offered in school, but more details can be found on the link below or by calling the immunisation team on 01743 730028.

The immunisation team will be in school on Friday 19th November to offer the COVID-19 Vaccine to children 12-years and over.

Please complete both sides and return to school by Tuesday 16th November.

Please note forms received after this time will not be guaranteed to receive a vaccine on the day.
