Dear Parent/Carers of Year 9 students

The immunisation team will be in school on Monday 16^th & Tuesday 17^th May to offer a “3 in 1” DTP Vaccination, School leaver Booster (SLB) for year 9 students.

The double-sided consent form has been given to your child(ren) today and can also be found here.

Please complete both sides and return to school by Thursday 12th May .

Please note forms received after this time will not be guaranteed to receive a vaccine on the day.
